How can I verify my vision benefits?
We can assist you with this verification process. When you contact our office please be prepared to provide the Primary Insured ID's number from your previous Block ID card. You can also contact Superior Vision directly.
What happens if I do not have a Block ID# or card?
We can still assist you. Please provide the Primary Insured Name, Date of Birth and Social Security Number for your account when you call our office.
Can I determine what my out-of-pocket amount will be for the exam?
Once your coverage is verified, we will be able to determine what your Co-Pay will be for your examination.
Do you take Block Medicaid?
This is the vision plan provided by Amerigroup. We are not a provider for Amerigroup at this time.
